What do I need to know before my child has a sports physical?

A parent or legal guardian must accompany all students under the age of 18 for sports physicals. You must bring a completed Sports Physical Preparticipation Form signed by a parent or guardian to be seen.

The child should dress comfortably in case the provider asks them to perform simple stretches and exercise.
